CARE’s in-home and community-based ABA programming provides a carefully designed behavior plan to teach appropriate behavior and independent skills to our clients.  Throughout programming, CARE behavior technicians will work with your child to ensure they are successful by using evidence-based practices and making data-based decisions.

CARE’s intake forms and contracts are available online for your convenience. Before starting with the intake packet, please take a moment to submit the New Client Information form. This form allows us to verify your benefits.

Once our program manager has contacted your insurance company to determine benefit eligibility and provided those details to you, please return to this page to complete the CARE Intake Packet.
